As a dietitian or dietetic educator, you are invited to participate in a two-phase study on Interprofessional Dietetic Education for Team-Based Primary Care. Phase 1 of this study involves completing a confidential online survey, and Phase 2 includes participating in the evaluation of the Interprofessional Dietitian Team-Based Primary Care eLearning Toolkit. You are now being asked to participate in Phase 1 of this study, completing the online survey. The survey will take between 15-20 minutes to complete.

All information entered by you in this survey will remain confidential. Answers will be anonymized and collated so that no individual response can be identifiable. Participation in this study is voluntary, and you may withdraw or exit the survey at any time before completion.

There are minimal risks to participating in this study, and potential benefits may include enhancing your knowledge and skills related to strengthening key practice competencies. The information gathered will help provide a better understanding of the experiences and needs of dietitians in primary care.
